NOMINATION FORMS

DONNA'S DREAM HOUSE offers free holidays to children and teenagers with serious or life-threatening illnesses, and also for the siblings of a child who has passed away in the last two years. Nominations can be made by anybody who knows the family concerned. The holidays are free - ie. there is no charge for accommodation, and daily meal vouchers are provided for a neighbouring cafe. There are kitchen facilities for you to prepare your own meals, and passes are provided to many of Blackpool's attractions. There is a special Teens' Centre with state-of-the-art facilities for those aged 14-24.

There are three downloadable nomination forms on this page.

To nominate a child with a serious illness, please use FORM A. 

To nominate the sibling(s) of a child who has passed away in the last two years, please use FORM B.

To nominate a teenager (age 14-24) with a serious illness, please use FORM C.

Simply click on the button for the appropriate form. There may be a short pause before anything happens, then you can either open the form and view it on screen, or save it to your computer. Next you should print it out, fill it in and post it to us. (The address is on the form).
